• Comprehensive in-person evaluation of elder’s

• Help family find appropriate care settings
• Large database of local residential placements
     for Assisted Living, Alzheimer’s &
    Dementia, Board & Care facilities

GERIATRIC CARE MANAGEMENT

• Skilled psycho-social assessment of the elder’s current capabilities and needs
• Coordinate needed community resources to support the elderly
• Help set up a Care Plan with the family
• Liaison with families and physician reporting progress
• Professionally assist with difficult decision making: driving issues, cognitive status,
    placement needs, hospice, etc.
• Long term planning of financial, legal and medical issues to prevent future problems
• Crisis intervention, counseling and support

RESIDENTIAL CARE FACILITIES

• 1:3 ratio, 24 hours a day
• Provide care for the elderly age 60 and over; including Dementia and Alzheimer patients.
